Computer & Devices - Report an Issue

Service Description

Device failures impact productivity.  Request assistance for university-owned computer equipment failures in your office, a podium computer or a computer lab, including kiosks.  Use this service to report broken or non-functioning devices that need to be fixed or replaced.

Examples of devices:

  • Computer or laptop won't boot or is exhibiting random issues
  • Keyboards, mice, monitors, docking stations, or other peripherals that no longer function properly
  • Tablets/iPads
  • Digital Signs & TVs

When filling out the form, be sure to provide the computer name or service tag information. 

How to find the computer/device name:

  • Windows devices:  right-click on Windows Start button and choose System 
  • Mac devices:  go to System Preferences>Sharing

NOTE regarding printer support: 

  • For Kyocera devices, please refer to the vendor sticker on the front of the printer.  Contact the vendor, providing the device Tag #, to report equipment failures.
  • For non-Kyocera devices, please use the Printer & Scanners - Report an Issue service.
