Using the Dynamic Forms SVSU User Portal

Purpose

Dynamic Forms has a user portal for accessing forms to be signed or viewing those already completed.

Policy/Procedure

The Dynamic Forms SVSU User Portal has 3 sections as described below:

Assigned To Me

A "To Do" list of forms that belong to the user signed in.

Pending Action

Forms the user is currently working on and need to complete.  To continue a form that you saved as a draft or that needs your signature click on the Actions button and Resume Form. 

You can also access Pending/Draft Forms by clicking on My Forms – Pending/Draft Forms at the top of the SVSU User Portal.

View Forms History

To view previously submitted forms click on View Forms History.  You can then search to find a specific form by any field, form, participant, etc. related to the form.  To view a form click on the PDF icon on the right. 

You can also access Forms History by clicking on My Forms – Forms History at the top of the SVSU User Portal.       

In Forms History, you can sort any column by clicking on the column header name.

Other forms

Forms that are available to fill out and submit are listed under each department.  For example click on the down arrow next to Saginaw Valley State University – Human Resources for a list of available forms.  To submit a new form click on the Fill out form button next to the appropriate form.

Helpful Tips

  • If you receive an email to sign/approve a form, the link will only work for you (or the group) that is required to sign and submit the form.
  • To get to the main Portal screen click on My Forms – SVSU User Portal.
